Care Co-ordinator and Social Prescriber
We have a designated Care Co-ordinator to help provide support for patients, including the elderly and those with long-term conditions, across health and care services. They will ensure that any services are patient-centred. The Care Co-ordinator will work closely with the GP and other primary care professionals within the PCN to identity and manage caseload of identified patients, makeing sure that appropriate support is made available to them and their carers and ensuring that their changing needs are addresses.
We also have a designated Social Prescriber available to help support patients with social issues. Social Prescribing is a way of supporting people with a wide range of social, emotional or practical needs, and many schemes are focused on improving mental health and physical wellbeing. Those who could benefit from social prescribing schemes include people with mild or long-term mental health problems, people with complex needs, people who are socially isolated and those with multiple long-term conditions who frequently attend either primary or secondary health care. The Social Prescriber will also help patients to address wider detriments of health, such as poor housing, debt, stress and loneliness.
If you would like to talk with our Care Co-ordinator, please ring the surgery on 01268 542 901 to arrange an appointment.
