PPG Sign Up Form

We are looking to start up our Patient Participation Group again. If you are interested in joining the group would you please complete the following form or paper form in reception and you will be contacted back. The group will meet virtually on teams between Monday to Friday 9.00 to 5.00pm. We plan on holding our Patient Participation Groups virtually on a 2-monthly basis.
